HOW TO PROCESS HEALTH CERTIFICATE IN BAGUIO CITY ?

1. Proceed to BAGUIO HEALTH CENTER located at T.Alonzo Street,  Baguio City.

2. Ask the person in charge in the front desk in the entrance section for the application form for the Health Certificate.

3. Fill up the form and attach 1 1x1 ID picture then return the form to the person who issued the form for signing.

4. Submit STOOL/FECES/TAE at the laboratory (basement floor ). Thumb-sized stool sample placed in a wide mouth plastic container with cover. 

* note: 

-Submission of Feces/Stool/Tae in the laboratory is until  10am only. 

-You can't defecate within the building so it's better to bring your feces in the Health Center and for less hassle. (You can defecate at home and submit it right away in the Health Center but make sure it's not more than One hour already. Cause it may affect the laboratory  result ) 

- You can buy the stool container in any drugstores or you can avail it also within the Health Center for 10 pesos.

-Claiming of results is around 1pm onwards.

- If your result is positive from parasites you need to undergo medication first and return for another laboratory test. 

5. RPR TESTING ( if required) 

6. CHEST-XRAY (basement floor ) 

(X-ray for normal findings is valid for 2 years )

*note: 

- You can proceed right away for the X-ray once your done submitting your Stool. 

7. Pay at the cashier (1st floor, room 104)

8. The signing of Doctor (1st floor, Room 105)

* requirements: 

1. result of stool exam and X-ray 

2. short nails 

9. Issuance/Renewal of HEALTH CERTIFICATE (2nd floor, room 204/sanitation).

* submit one  1x1 ID picture for new applicants to be placed in your Health Certificate) 

*If you have no problem with the lab result then you can get your HEAlth certificate within the day.
